The template for the cards is in card.tex. You just need to replace my details with yours in the line that begins "\newcommand{\mycard}..." near the top of the file, as indicated in the comments. Here is the result in PostScript form.
You can print it on thin card which you can get from the General Office (if you ask nicely). I often have a few spare sheets anyway. But take care to put the printer off-line & check that your job is first in the queue before feeding the card in. (Otherwise you are quite likely to get the front page of someone else's printout on your nice piece of coloured card.) Also, send them to the single-sided print queue, otherwise they get a bit bent. There is a guillotine in the CVSSP photocopier room.
Disclaimer: This card does not conform to the rules prescribed by the University Thought Police, who have very strict rules about what these things should look like. (However it does have the distinct advantage that you can always have an up-to-date card, with all the details correct.)
